How does IRMAA work?

Most people pay the standard Part B premium. If your Modified Adjusted Gross Income (MAGI) from two years ago is above a set threshold, Social Security adds an Income-Related Monthly Adjustment Amount on top of the standard Part B premium, and a separate, smaller surcharge on top of your Part D plan premium (if you have one). There are six income tiers; the higher your MAGI, the higher tier you fall into. IRMAA is reassessed every year using your tax return from two years prior.

Social Security, not the Centers for Medicare & Medicaid Services (CMS) and not your insurance agent, determines and bills IRMAA. You will typically pay it as part of your monthly Part B premium (often deducted from Social Security) and, if it applies, as a separate charge for Part D.

How do I appeal an IRMAA determination?

If you believe Social Security used the wrong or outdated tax information, you can request a reconsideration. If your income has dropped because of a qualifying life-changing event, such as marriage, divorce, death of a spouse, work stoppage or reduction, loss of income-producing property, loss or reduction of pension income, or an employer settlement payment, you can file Form SSA-44 to ask Social Security to use more recent income instead of the two-year-old tax return.

Form SSA-44 is available at ssa.gov/forms/ssa-44.pdf, or you can file it online, by fax, by mail, or at a local Social Security office. Instructions and the online option are at ssa.gov/medicare/lower-irmaa. If you disagree with a reconsideration decision, you can request a further appeal; your determination notice explains the exact deadline and process.

Common questions about IRMAA

What income does IRMAA use?

Your Modified Adjusted Gross Income (MAGI) from your tax return filed two years before the current year. For 2026 premiums, that is your 2024 MAGI. MAGI is your Adjusted Gross Income plus tax-exempt interest.

Does IRMAA apply to Medicare Advantage plans?

IRMAA is added to your Part B premium (and to your Part D premium if you have drug coverage, including drug coverage bundled into a Medicare Advantage plan). It is billed separately from your plan premium, usually through Social Security.

Will my IRMAA change every year?

It can. SSA reviews your MAGI from two years prior each year, so a change in your income can move you into a different bracket, or out of IRMAA altogether, the following year.

Is this calculator the same as my official IRMAA determination?

No. This is an educational estimate based on the published 2026 bracket table. Your actual IRMAA is determined by the Social Security Administration using your filed tax return and is stated in your official determination notice.

What if my income dropped after a life event, like retiring?

You can ask SSA to use more recent income instead of the two-year-old tax return by filing Form SSA-44 for a qualifying life-changing event. See "How do I appeal an IRMAA determination?" below.
